How can we help you help your patients? Lakeshore Sports Physical Therapy, PC, was founded in 2000 by Angela Wilson Pennisi, PT, MS, OCS, and rebranded as PhysioPartners in 2016. At PhysioPartners, we cultivate collaborative professional relationships with referring physicians for the benefit of the patient. Our therapists are experienced and engage in thoughtful clinical decision-making and keep you informed when the situation warrants.
Communication of Progress
Upon completion of the initial examination, our findings of impairments and functional limitations are communicated in a timely, typed letter. If the patient is progressing as expected, you will receive a follow up letter every four weeks or upon return visit to your office. All communications are formatted for simple comparison of the patients status.
Evidence-Based Treatment Approach
We utilize an eclectic treatment approach, combining manual therapy, prescriptive exercise and education to speed recovery and prevent recurrence. We consistently refer to the literature and consider the evidence when planning our treatment and utilize objective measures to gauge progress. We track patient outcomes on an ongoing basis and compare our performance to external and internal benchmarks. Click here for a listing of our specialized treatment programs, including McKenzie Method® for treatment of the spine and Vestibular Rehabilitation.
For your patients requiring urgent care, we offer appointments scheduled within one business day to maximize the patients response to treatment and reduce recovery times.
We expect full participation from our patients in their recovery and respect the time they are committing to their care. Each of your patients will receive one-on-one consultation and treatment during their visit. We make every effort to accommodate our patients schedules with appointments available from 6:30 AM to 7:30 PM most days and the availability of Saturday visits.
In order to promote carryover of recommended therapy and exercise at home, we have utilize a unique exercise instructional system utilizing written, audio and visual cues that can be accessed over the Internet. Each patient views only the exercises and instructions prescribed by his or her therapist, and the program is updated as their needs change. Patients without access to the Internet receive individualized written and pictured instructions.
Insurance Network Participation
PhysioPartners offers both in and out of network providers and treatment options. Your patient may choose to see a PhysioPartners Premier provider with limited insurance participation who will typically spend 45-60 minutes with you one-on-one every visit.
If seeing an in-network provider is important to your patient, they should schedule with one of our PhysioPartners fully participating providers who will always spend at least 30 minutes with them one-on-one every visit and work with our qualified team to help you make the most of your patient's therapy time.
Either way, you can ensure that with one-on-one consultation and treatment included in every visit, your patient's time is used more efficiently and focused on addressing the root cause of their problem.
PhysioPartners has providers who participate with the following insurance networks:
PhysioPartners Premier providers participate with Blue Cross Blue Shield PPO, Medicare and Point of Service. PhysioPartners and PhysioPartners Premier providers also accept all out-of-network benefits. PhysioPartners and PhysioPartners Premier cannot bill an attorney for physical therapy services.
Please call (773) 665-9950 for up-to-date participation information and to find a physical therapist who participates with your patient's insurance.
